If you’d like to mail in your request, you can access the request form here. Please print the form, fill it out, and send it to the abovementioned address.
You’ll need to know the accident’s date, time, and location, and should include the case or incident number if you have one. Usually, this is provided by the responding officer at the accident scene.
Finally, you can use this portal to place your request online through the state’s Government Records Access and Management Act (GRAMA) system. You will need the same information, but your request will be received more quickly with an online submission.
