How to Obtain a Copy of Your South Salt Lake Police Car Accident Report

You can use the online form or a printable form that can be dropped off at the South Salt Lake Police Department. The physical address is:

You should include your contact information (name, address, phone number, email address) and a detailed description of the record you need. The best way to help the records clerk find your report is to use the case number, which is located on paperwork provided by the responding officer.

You should also explain the location, date, and approximate time of the accident.

Car accident reports are not public under Utah laws, so you should include your involvement in the accident. Options on the printable form are:

  • Requestor was involved in the accident
  • Parent or legal guardian seeking report for a minor involved in the accident
  • Legal guardian of an incapacitated adult seeking a copy of the report
  • Requestor owned a vehicle involved in the collision
  • Insurance company

Here are just a few examples of situations where you may need qualified legal advice:

  • Mandatory PIP coverage in Utah is limited to $3,000 (although you can purchase a larger policy). With rising healthcare costs, many people find that their medical expenses quickly overtake the $3,000 limit. Remember, even if your initial hospital bill was below $3,000, follow-up appointments, physical therapy, and other care could create expensive medical bills later. Once your medical costs are over $3,000, you will need to make an at-fault claim, which is much more complicated.
  • If you suffer permanent disability, disfigurement, or impairment from your injuries, an at-fault claim will be necessary.
  • You can file an at-fault claim yourself, but these are heavily scrutinized by the insurance adjuster. It’s not unusual for the adjuster to blame the injured party for being fully or partially responsible for the collision. Even if they believe you are only 10 or 20 percent at fault, this could reduce your settlement by thousands of dollars. If the adjuster decides you are at least 50 percent at fault, they can deny the claim entirely.
  • While your PIP claim can’t be denied based on fault, there are other reasons it could be rejected.
  • Have you already filed an insurance claim and received an offer from the insurance company? In some cases, we find the adjuster has overlooked or underestimated some damages, and the final payout should be higher.
