The North Salt Lake Police Department accepts records requests in person, by mail, or by fax. You can find the form online or fill it out at the police department.

The police department is situated on the first floor and is open 7 AM to 5:30 PM, Monday through Friday. They are closed on holidays.

If you make your request in person, the report is often available the same day. However, in a few situations, the report may be unavailable for some reason.

If this happens, a Records clerk will arrange to call you when it’s ready.

Alternatively, you can print the online form and mail it to the address above or fax it to (801) 335-8679. Please be advised that your report will not be mailed or faxed to you.

When it’s available, a Records clerk will let you know and you will need to pick it up in person. Remember to bring your photo ID, which is required to obtain a report.

How Much Does a Police Report Cost?

Most reports are between $15 and $25, with an additional charge if you want photos or video from the scene (when available). The Records clerk will let you know the cost when they locate the report and confirm it can be released to you.

Who Can Request a Police Car Accident Report in Utah?

Under Utah’s Government Records Access and Management Act (GRAMA), most police records are considered private, protected, or controlled. Accident reports will only be released to people who were involved in the accident or are representing an involved party.

Involved parties include:

  • Drivers
  • Passengers
  • Anyone injured in the accident (passengers, pedestrians, bicyclists, etc.)
  • Owners of property damaged in the accident (vehicles, buildings, etc.)

Representatives of involved parties are usually either attorneys or insurance agents. However, if you are the parent or legal guardian of an involved minor, you may seek a copy of the police report on their behalf.

What Information Is Required On the Car Accident Report Request Form?

The request form is fairly brief and asks for the following:

  • Your name.
  • Your contact information, including an address and phone number.
  • Your ID number (driver’s license, state ID, passport).
  • The document you’re seeking and the case number, if you have it.
  • The location of the accident (mention the closest cross street if it wasn’t near a specific address).
  • The involved parties and their dates of birth.
  • A brief explanation of your involvement in the collision—driver, passenger, vehicle owner, parent of involved minor, etc.
  • The reason for your request—for instance, you need more information or need the report for an insurance claim.
